...I feel like the biggest loser for asking this. (I pretty much am!!)

I made this icon a while ago, from this pic, but I cannot for the life of me remember how to get that effect. I've been trying it in PS...but I can't get anything. xD I feel so stupid! Any help is appriciated. :3
