Made with photoshop 7. Icon is up for grab, but with credit, please. Please do not make the exact same icon, but try to make your own. *smiles* Comments are always appreciated.
So we will start with this base.

No auto contrast is necessary, as the contrast is already good. To make it more sharpen, I made an unsharp mask ( 70%. ).

I do want her skin to be soft, though, so I used the blur tool on it a bit, making sure not to touch to the "blood" or the lips.

See? a little softer.
There is a tricky part. I want to make her skin look pale, but when I tried random things, it also made her lips look too pale. So here is what I did. I duplicated the base and set the new layer on multiply. Erase everything except her lips. It might look way dark right now, but it will come clearer later. Erase the "blood" too, as it will look too dark in the final version.

As I said, right now, it looks odd.. but wait. :)
Duplicate the base 4 times, put them all on top of the multiply layer, and set them all on soft light. Desaturate them all except the one right on top of the multiply layer. So, it should give you 3 desaturated ones, and 1 under them with the normal colors. ( Image - Adjustement - Desaturate ).

We are almost done. Now it is time to add the text that you want.

Brushes if you want.

And the border.

There. Now, Miss Jolie is all looking vampire-ish! ( but the effect can work for simple pale skin too. hehe )
