Don't know what to call this >.

Okay, I made this icon off of a tutorial.
I don't remember how I made it, nor did I bookmark the page.
I cannot remember who it was by either. I have a bad short term memory.
I was wondering if anyone could point me in the direction of the tutorial or
just help me out.
