Brush Instruction Please!

I've looked through the memories to see if a question like this has already been answered but to no avail. Can somebody please instruct me on how to properly use a brush like this one (absolutetrouble.com)? I have no idea how to get the icon picture into the black part of the brush, and have it look realistic.
