Locating a PS tutorial

I hope that someone out there could help me. I found a tutorial today, one that I very much liked and have since lost it. I've browsed through the massive memories and my browser's history and nothing's jumping out.

She used a specific base by icons_with_love: , put her image of some actress on top of it cropped to 100 X 80, and had a layer filled with a tanish/gray color and then used a brush made by calixa to erase part of that and had that as a screen layer. The image was then copied to the top and screened again. I know that the text was "are you really happy?". Would somebody know who did this tutorial? I'd love to be able to properly credit.
