1) Base made out of a picture from KellySource.com, grayscaled using Image > Adjustments > Channel Mixer > with THESE options

2) I selected a part of the icon, particularly from her face, press Ctrl + J to duplicate it, move to the left side of the icon.

3) Add a small line border to the side of the image, 6 pixels in width, just like this:

4) Add a small text brush, I got this one from
terra_katta, turned 90 degrees.

5) Adding some sets of accents ...
Rectangular boxes in gray colors

Perfect square done with the marquee tool holding shift, added a black border o 5px, added her face inside.

Another, just smaller, perfect square in a dark gray (#696969) color

Last brush accent of white, from
awmp, I think?

6) Lastly a light texture, I don't remember where I got it, I'm sorry, please check my resources blending mode Screen 100%, turned Horizontally and Vertically

7) Added some contrast to the base, and DONE!
