This is my first ever tutorial, so wish me luck!
We'll be turning this
into this 
1. OK, first I took this screencap of the lovely Mirage from The Incredibles. The cap is
scofo's and the rest of the caps can be found here.
2. I then crop the image into a 100x100 square, using the crop tool, and I get this

3. Obviously, that's far too dark. To brighten her up a bit, I go to Images --> Adjustments --> Levels and slide the arrow to the left, until it's a suitable brightness. I duplicate the image and sharpen once. Flatten those two layers. I've now got this

4. Now, for some colour. I take this gradient by
crumblingwalls and paste in on at Soft Light, 58%

5. Next, take this gradient, also by
crumblingwalls and set it at Pin Light, 90%

6. That leaves me with this

7. Looking good! Although, it's a litte dark. So, go down to the base layer, duplicate it and drag it to the top. Desaturate it (that's Image --> Adjustments --> Desaturate) and set it to Screen, 47%. Adjust the Brightness and Contrast of this layer to suit you, and you'll end up with something that looks like this

8. Now we want to duplicate the image (Image --> Duplicate). Then, create a new document (100x100, white). Take one of your images (either your original or your new duplicate) and flatten it (Layer --> Flatten Image). Go to Select --> All to select the whole icon, then Edit --> Copy to copy it all. Then, go to your new blank document and paste it in there, by going Edit --> Paste.
9. Now, we want to move the image to the right slighty, by using the move tool. We now select the Rectangle Tool (colour white) and draw a rectangle on the right hand side, to leave you something like this (although you can't see the outline of the box, obviously, as the screen is white here)

10. Now, move over to the duplicate you made of your coloured icon and select the base layer. Then select the Rectangular Marquee tool, and select a portion of the blue area on the right hand side of the image. Copy this selection, and paste it into your new icon, moving it to the right. Desaturate it, adjust Brightness and Contrast, until you have this

11. Almost done! Now, we're going to add text. I used the font "Eurostile" which can be found at DaFont. I chose white as the text colour and typed "MIRAGE" onto the icon. I left the layer mode at Normal. Next, we take the Move Tool, and drag the text around until it's vertical. Position it just over the white line, and we get the finished icon!
