make an icon like this 
okay the picture im gonna use is this one of lindsay lohan
crop it to 100 x 100
duplicate the base and go to image -> adjustments -> hue/saturation and set the saturation to -65 and set the blend mode to overlay and set the opacity at 65%
make a new layer with this gradient
and set it to exclusion
now duplicate the base and drag it to the top and set it to pin light at 50% opacity
now duplicate that layer and set it to 100% opacity and hard light
duplicate that layer 2 times.. the 1st duplicated layer (the middle one) leave it alone... and the 2nd duplicated layer (the top) set to overlay at 100% opacity
then duplicate again and set to screen at 100% opacity
make a new layer with this gradient
and set the blend mode to lighten and the opacity to 75%
now duplicate the base again and drag it all the way to the top and set it to darken
make a new layer and fill it with this color " FFCC99 " (its a peachy color) and set it to darken and the opacity to 50%
make a new layer with this gradient
and set it to multiply, leave the opacity at 100%
make a new layer and fill it with this color " F2EEDE " (its an off-white color) and set the blend mode to multiply
make another new layer and take this gradient
and set it to overlay and 285 opacity
then i added this border
i erased the white part with the magic wand tool and set the blend mode to soft light
again, make another layer and use this gradient
and set it to hard light
on another new layer use thsis gradient, again,
and set it to pin light and 80% opacity
then i used this gradient
and set it to soft light at 20% opacity
we're almost done it should look something like this so far
use this gradient next
and set it to color burn and opacity to 35%
now duplicate te base again drag it up to the top set the blend mode to hard light and the opacity to 20%
and to brighten it up a little i made a new layer and filled it with this color " CCCCFF " (a very light purple color) and set it to soft light at 10% opacity
and the final result is
it came out a little dark by her face, so i would either turn her picture before starting... it came out a little better on the other icon i made, but i cant post that now b/c im entering it in a contest... ill post that one when the contest is over...
