Made with PSP8 but I'm sure it can be translated into other programs.

Alright, i started with my base

then I DUPLICATED it. sharpened it once. Then i set the duplicated layer to SCREEN to brighten it up.

I then merged the layers.

Make a new raster layer. fill it with this gradient:

Then i set the layer to screen. 100 opacity

THEN i went back to the background, duplicated it and dragged it to the top. I then set it to OVERLAY 72 opacity.

Looks a bit crappy eh?
I merged the layers, then i went to the soften tool.
Size of brush: 8
Hardness: 50
Opacity: 61

I smoothed out the sharp edges and got this:

Then i just added some lovely text:
First I rotated it to the left 90 degrees.

Added the text: "this isn't me"
in Arial Black on Bold and Italic. Size 12
Then i went to Effects->3D Effects->Drop Shadow.

The properties should be:

Vertical: 1
Horizontal: 1

Opacity: 100
Blur: 0

the colour should be black.

Then i just rotated it 90 degrees to the right.
