gif in the image.

i've searched the archives but nothing is really working.
i'm looking to incorporate a gif into the image like the
one you see under the cut.

any help at all would be amazing, i've been trying for about
a month and a half now. i'm using photoshop cs3 portable. i
also have windows vista. i don't have image ready..
