FE [Import by URL] Generate project name automatically, improve accessibility

It has been reported that the previous Haml-based form for Repository Import had the feature of suggesting a project name based on the git URL.

Investigate restoring this feature so as not to degrade the user experience with the improved form.

In addition, investigate the report that the username and password fields are auto-filled even when the user did not request it.

Acceptance Criteria

• User can tab through fields
• Generate the project name from the url
• The sign in fields do not provide autofill automatically, if that is currently the case.
