Summary

If the agent is disabled, agent commands still appear in the command palette.

This also affects copilot and assistant commands.

Description

Steps to reproduce:

1. Disable the agent ("agent": { "version": "2", "enabled": false })
2. Open the command palette and type agent

Expected Behavior: No agent commands should appear in the command palette
Actual Behavior: Agent commands appear in the command palette
