Updates the server auth metadata discovery helpers to support the path insertion styles in 4.3 Authorization Server Metadata Discovery that were added/clarified in MCP 2025-11-25:
Includes some new tests of the helper, to ensure all supported discovery URLs are checked (if necessary).
