[Bug]: Username and Password pre-filled on new User Account creation #782

Describe the problem
probably correlated with #769
When I want to add a new user-account, the username and password fields are already filled with the username and password of the account currently logged in and used to create that new user.
Steps to reproduce
login, create new user.
Poweradmin version
4.0.1
Database
MySQL
