Description

When running commands via exec tool, sometimes get error: 'missing tool result in session history; inserted synthetic error result for transcript repair'

This happens frequently when:

• Running commands via exec tool
• Gateway restarts
• Long-running commands

Environment

• OpenClaw: 2026.3.2-beta.1
• macOS 15.6
• Node 22.22.0

Expected Behavior

Should not show synthetic error messages

Actual Behavior

Shows: '[openclaw] missing tool result in session history; inserted synthetic error result for transcript repair'

Steps to Reproduce

1. Run gateway restart
2. Run any exec command
3. See error message

Additional Context

This is cosmetic but confusing to users. The gateway continues to work normally.
