refactor: extract ACP manager runtime handle cache
Extract ACP manager runtime-handle cache ownership into a dedicated helper. Keeps the session manager focused on lifecycle orchestration while preserving cached handle reuse, close/clear, idle eviction, matching, and observability behavior. Proof: focused ACP manager runtime-handle/runtime-config tests, narrow oxlint, pnpm check:test-types, autoreview clean, PR CI green.
