Fresh install, recent version, Authorize.net module and/or COD module enabled.
I get a token error on step 2 of checkout with both modules

Undefined index: company_id in catalog/controller/checkout/guest.php
Undefined index: tax_id in catalog/controller/checkout/guest.php
