You can now set column headers using NVDA+shift+c, but this is specific to screen readers. Word allows you to set the top row as a header row using the "Repeat as header row at the top of each page" check box on the Row tab of the Table Properties dialog. This is visible visually and is also tagged in exported PDF documents. NVDA should support this too.
