Updated PgSqlDriver and SqlsrvDriver to use explicitly
PDO::FETCH_ASSOC- tests were failing because of both ASSOC & NUMERIC fetching.PgSqlDriver::getColumns() now returns columns of materialized view too.
Improved @otuhacek's commit (#286) and covered by test.
