Steps to reproduce the problem

1. Open provided PDF in Native viewer (Adobe Acrobat)
2. Open provided PDF in Chrome browser
3. Open provided PDF in Firefox or PDFjs viewer

What is the expected behavior?

What we expect is when sample document is opened in pdfjs it should display checkbox with selected state as compared with Adobe viewer or Chromium browser

What went wrong?

Observation : Only in Firefox or PDFjs viewer I cannot see checkbox are pre-checked , which should be pre checked as attached document

Note : At times if I update sample test document there is a chance that latest checkbox state appears in pdfjs render.
