Sometimes vscode seems to get confused and is unable to interact with comments.

It seems like when you open a comment from the comments panel sometimes it will open the file at the revision that the comment was made on in read-only mode, and sometimes it will open it at the current revision. Sometimes when it opens the comment at the current revision, the comment will not be on the correct line and in that case the "resolve conversion" button does not work.

I think it might be when a comment should be marked outdated but for some reason isn't.
