MCP: Failed MCP tool execution shows with checkmark and isn't expandable

Steps to Reproduce:

1. Invoke a tool that will fail, like using the _perplexity_ MCP without docker being started
2. Tool call will show up as ✅, but can't be opened to see the input or error
