These changes are preparing the project for a final 4.0 release:

• Add a new 'messageformat/cst' entry point, and move all CST APIs to it.
• Export all data model types in a new Model namespace.
• Define a MessageFunction type instead of a MessageFunctions type.
• Use locale, source argument order in @messageformat/fluent.
• Documentation updates.
