Allow the usage of ranges to download episodes from a podcast.

Thanks @dstpierre for adding this feature.

This basically only includes a fix so that file names are sanitized (extensions no longer include entire url params)

Some fixes with regards to sorting podcasts + configurable concurrency.

• Some cleanup
• No longer invoke a player from the app, but provide stdout to pipe into other players
