Describe the bug
Attempting to create a new notification transport for a webhook does not bring up the UI needed to specify the webhook url.

To Reproduce
Steps to reproduce the behavior:

1. Click on Events > Notification Transports
2. Click create
3. Name the transport and select 'Webhook (generic)'
4. Note that no UI for entering the webhook URL is presented.
5. Click 'Create' and get an error message which is blank

Expected behavior
I would expect the UI to specify the webhook URL to show, and the transport to be successfully created.
