Submission attachment is shown as downloadable even when it's missing

Steps to reproduce the problem

Make a submission with a missing attachment (for example, in Enketo, annotate an image, scale down the canvas, submit), view the submission table

Expected behavior

If the submission has state missingAttachment, issue HEAD requests for each of the expected attachments and display a red x with a tooltip explanation for any that are missing. See @matthew-white's comment at #706 (comment)
