Describe the bug
Github-copilot (and maybe others) doesn't refresh token during tool call.

To Reproduce
Steps to reproduce the behavior:

1. Login to github-copilot
2. Let LLM call tool
3. Wait with approval (so token expires)
4. LLM response status: 401 body: unauthorized: token expired

Expected behavior
Token is updated during tool calls
