Add setting to initiative types with list of extra user fields to be collected in signature #4652

This issue is part of the EPIC: Signature process of an initiative #4644

  • For each type of initiative in admin include a list to set which extra user attributes has to be collected signing an initiative:
    • Name and surname,
    • DNI / NIE / Passport
    • Date of birth
    • Postal code.
  • Also add an optional text box with legal information to be displayed in the extra user attributes form.

I have a question: The DNI is included in these extra fields? The DNI should always be asked to the user and used in the security mechanism to allow signing and avoid duplicated votes. On the other hand, in the list of all signatures in PDF format we should include only the extra fields enabled to be requested to the user or the DNI should be always included, even if it's not enabled in the administration panel?
