Add README.md and update documentation

- Add README.md with usage instructions, examples, and error reference
- Update CONTRIBUTING.md with build instructions and Unlicense CLA
- Update DESIGN.md to match implementation (GPT-5.2, dependencies, etc.)
- Mark Task 8 as complete in TASKS.md
