Fixes the following bug: When using '--segment-list' mp4-dash.py creates N segments starting with seg-1.m4f but puts N+1 segments in the mpd starting with seg-0.m4f which doesn't exist.
Adds cenc:pssh element inside of widevine's ContentProtection tag when using the flag '--widevine-header'.
