The Foundry endpoint authenticates API-key requests with the x-api-key
header. Previously this worked by accident: the base client emitted
X-Api-Key from auth_headers. When auth_headers was emptied to stop a
stray ANTHROPIC_API_KEY environment variable from leaking to the
Foundry endpoint, API-key requests were left with only the api-key
header, which the endpoint rejects with a 401.

Attach x-api-key (alongside api-key, kept for backwards compatibility)
in _prepare_options. This branch only runs when no Azure AD token
provider is configured, so the key is always the explicitly provided
Foundry key, never an environment ANTHROPIC_API_KEY — the leak fix
still holds.
