Preserve hold timer for known sessions with fresh tags so transient capture failures in hysteresis don't immediately deactivate them (P2). Rename ShouldFallbackForStaleTag → HasRecentWindowActivity for clarity. Add ScoreThreshold asymmetry comment in FreshTagVisibleActivity. Co-Authored-By: Claude Opus 4.6 <noreply@anthropic.com>
Fix FreshTagVisibleActivity dropping state from updated map when capture fails, causing re-seeding every scan. Add clarifying comments for: first-observation active blip, hold-timer preservation semantics, double capture-pane cost, score-cap/seed interaction, and scanIndex test coupling.
