Feature Description

Some diseases in LU are only notified by doctors by phone (e.g. IMI). In order to be able to track the declarations made by phone, the simplest way is to add functionality for users to edit the existing "Notified by" box.

The existing Notified box currently can only be updated via doctor's declarations via the external msg directory.

The options for filling out the notified by box should match the options filled out by the external DD, with an additional new option to show Notified by: Phone or External Message
The notified box should only be editable manually if no doctor's declaration external message has been received, there should be no modification possible for notifications coming from external messages
Users should be able to edit the phone notification once it has been saved, in case they have made errors

Added Value/Benefit

LU users will sometimes receive doctors declaration by phone for diseases such as IMI. It would benefit their work to be able to display this information in SORMAS for future users checking in on cases.
