Updates the issue and PR templates.

• Puts the descriptions into <!-- --> blocks (People seem to have constant formatting issues)
• Changes the last part of the Bug Report template to ask for the /papi dump output
• Adds Closes N/A to the Pull Request description and hides the "Please Read" section into a <!-- --> block
