Some configurations for BTM financial models include inputs for land lease costs, but cashloan does not calculate land lease costs:

Fixing this requires:

1. Remove the 'Operating Costs Land Lease' form from the startup.lk script for these configurations
2. Remove land lease cost inputs from defaults for these configurations

To Reproduce

1. Create a case for one of the above configurations.
2. Note land lease cost inputs on Operating Costs page.
3. Click Simulate.
4. Note no land lease cost outputs in "Operating Expenses" section of cash flow.
