Improved ADFS adapter handling of HTTP 403 responses: when errorCode=invalid_callback_url, bypass is now forcibly disabled to prevent authentication bypass caused by callback URL misconfiguration. Bypass behavior remains available for genuine API unavailability when enabled by configuration.
