GitHub pull request view does not show up in side bar

• Extension version: 0.2.1
• VSCode Version: 1.28.1
• OS: macOS High Sierra 10.13.6
• GitHub Enterprise 2.14.5

Steps to Reproduce:

1. Install GitHub Pull Requests extension.
2. Reload VS Code.
3. Follow all the sign-in steps to grant GitHub Enterprise permissions to the VS Code extension.

GitHub pull request view still does not show up in side bar.
