Related to #3656 (comment), and a cut-down version of #3607. If a user just wants to replace the Run/Debug links for all tests/groups/test files (eg. there is no path on the custom CodeLens), we don't need new profiles (we already have Run/Debug) and we don't need to vary them per-file (so it doesn't interfere with running multiple suites in the same process).
