Repro steps
Do azd init -t todo-nodejs-mongo
Expected
Should see the prompt for environment name just once
Actual
After entering the environment name the prompt (and the chosen environment name) is shown again above the next prompt (for location).

azd compiled from sources, commit 1b57d3d as of morning 8/5/2022- Powershell 7.2.5 on Windows 11 21H2 22000.795
- Screenshot from Windows Terminal app, but I have observed this inside VS Code terminal as well
