Rapid treatment responses are crucial to achieve effective outcomes. Simulation education has been shown to improve timely decision-making related to reperfusion treatments, stroke pathways, organisational aspects of multidisciplinary stroke management and clinical outcomes. An effective stroke response involves interdisciplinary collaboration, with team communication and effective cooperation being especially essential. These are key learning objectives of simulation training.</p>
<p>The European Stroke Organisation (ESO) Simulation Education Committee was established to advance stroke education through simulation and to broaden access for physicians and paramedics involved in stroke care across Europe. Over recent years, multiple simulation workshops have been successfully delivered at ESO Congresses. However, wider implementation requires structured training for instructors to enable dissemination at the local and regional levels. The present Train-the-Trainer workshop, therefore, aims to expand simulation training opportunities by equipping participants with the skills to design scenarios and plan and conduct simulation programmes tailored to their specific clinical environments.</p>
<p>The target audience of our workshop comes from multiple disciplines (neurologists, stroke physicians, neuro-interventionalists, nurses, etc.), enabling multidisciplinary and harmonisation of treatment paradigms. Our program is designed to provide attendees with advanced skills in the pedagogy of simulation to improve educational experiences for their institution, region or country. Participants will be enabled to set up their own simulation program and train local acute stroke care teams. It is anticipated that the flow-on effect will improve care delivery and clinical outcomes for patients with stroke.</p>

<p>This highly interactive workshop is designed for neurologists, neuroradiologists, interventionalists, nurses, and other health professionals involved in acute stroke care who want to develop or strengthen simulation-based training programs in their institutions.</p>
<p>The emphasis is on interdisciplinary team performance, communication, and workflow optimisation in acute stroke care. Purely catheter-based or technical interventional simulation procedures are not part of the programme.</p>

The 12th ESO EAST Workshop was held on 5 May 2026 in Maastricht, the Netherlands, traditionally organised a day before the European Stroke Organisation Conference.  It followed the Joint Stroke Action Plan for Europe National Coordinators (SAP-E NC) &amp; ESO EAST Meeting and gathered ESO EAST Country Representatives, the ESO EAST Steering Committee, ANGELS Initiative representatives, the RES-Q team, ESO Head Office representatives, invited guests, and industry partners.</p>
<p>The programme focused on key developments in stroke care across the ESO EAST region, with mainly sessions structured around an introductory presentation followed by country experiences and discussion.</p>
<p>The first session, chaired by Dmytro Lebedynets and Alexandras Vilionskis, addressed mechanical thrombectomy issues in the region, focusing on training and implementation. An introductory presentation by Dmytro Lebedynets, based on the ESO EAST survey, provided an overview of current practices and challenges, followed by country presentations illustrating different stages of development and service organisation.</p>
<p>This was followed by the RES-Q registry session, chaired by Cristina Tiu and Robert Mikulik. Robert Mikulik opened the session with an overview of new developments in the RES-Q registry, with a particular focus on intracerebral hemorrhage (ICH) and quality monitoring. Country contributions highlighted experiences with implementing ICH guidelines in clinical practice.</p>
<p>The session on emerging leaders in stroke, chaired by Alexander Tsiskaridze and Hrvoje Budinčević, presented selected projects reflecting ongoing innovation in the region. Presentations by Marieta Peycheva and Rytis Masiliūnas demonstrated approaches to improving stroke care through digital solutions (virtual hospitals) and national system-level initiatives.</p>
<p>The final session, chaired by Zuzana Gvodinova and Michal Karlinski, focused on neurorehabilitation. Michal Karlinski introduced the session with results from the ESO EAST survey, followed by country presentations addressing current practice and remaining gaps in achieving optimal standards of care in neurorehabilitation.</p>
<p>Discussions were conducted in an amicable, constructive atmosphere, continuing the long-standing tradition of collaboration within the ESO EAST community.</p>

For four weeks, I worked in the Hyper Acute Stroke Unit (HASU) and had the opportunity to observe its daily workflow, participate in clinical discussions, and learn from a highly organised multidisciplinary team.</p>
<p>The HASU has 20 beds, and every morning the team &#8211; including neurologists or geriatricians, residents, nurses, physiotherapists, nutritionists, and speech therapists &#8211; meets for a structured pre-ward round discussion. Each member documents important clinical, laboratory, and radiological information for all patients. After this meeting, doctors review updated imaging and laboratory results, while a neuro-radiologist joins the team on Mondays to discuss CT and MRI findings. During the ward round, patients are examined, results are reviewed at the bedside, and family members are informed about the treatment plan. After the round, the team discusses new information and finalises the plan for investigations and treatment.</p>

<p>What impressed me most was the high level of involvement of junior doctors and residents in every stage of patient care. Each team member has a clearly defined role, from the emergency department through discharge, which ensures smooth coordination and efficient workflow. The thrombolysis call system is another strength of the unit. When the on-call team receives the alert, two residents and one stroke nurse arrive in the Emergency Department within 10 minutes. Their actions follow clear protocols and are supervised by the on-call neurologist, contributing to a rapid door-to-needle time</p>

<p>After returning to Georgia, I delivered a presentation to my department about the HASU model, which does not currently exist in our healthcare system. I explained how the unit is organised, how daily work is structured, and how thrombolysis is managed in a timely manner. I emphasised that neurologists and geriatricians are directly involved in interpreting neuroimaging and making important treatment decisions, while in my country, we are more dependent on the radiology team for these evaluations. I also discussed differences in acute stroke management, secondary prevention strategies, and ongoing stroke research at St. George’s Hospital. My colleagues asked many questions, which showed their interest and<br />
eagerness to improve stroke care.</p>
<p>Overall, this experience strengthened my clinical skills and deepened my understanding of stroke care organisation. It inspired me to apply some of these practices in my home institution, particularly regarding structured teamwork, efficient thrombolysis, and multidisciplinary collaboration. I am grateful to ESO for this opportunity and to the entire HASU team for their openness and support. I would like to especially thank my supervisor and Head of Department, Dr Anthony Pereira, for guidance and encouragement during my stay.</p>

During my stay, I was fully integrated into the daily activities of the Neurology Department under close team supervision.</p>
<p>My primary interest was focused on the diagnostics and treatment of stroke patients, although the scope of clinical exposure was broad and diverse. Each working day began early with a departmental meeting, during which events from the night shift were discussed. This was followed by morning rounds, where patients were examined, clinical notes were taken, and laboratory tests and diagnostic studies were ordered and, in some cases, performed. Subsequently, another team meeting took place to review the findings from the rounds and discuss management plans for each patient. After lunch, imaging studies were reviewed in close collaboration with the radiology department. The workflow was dynamic and well structured; the day passed swiftly and was notably busy &#8211; the majority of routine clinical matters were well taken care of during the first half of the day.</p>

<p>Throughout my stay, I observed numerous differences between the German and Georgian healthcare systems. These differences allowed me to encounter patient populations and clinical scenarios that I would not usually see in my home country, where my practice is predominantly focused on inpatient care. I was exposed to a wide range of diagnostic techniques and participated in the differential diagnosis of rare neurological disorders and conditions.</p>
<p>One of the most memorable and inspiring experiences was the use of neurosonology performed directly by neurologists. While neurosonology itself was not new to me, in my home country, it is typically the domain of another specialty, namely a subspecialty of radiology. Observing how flexibly and efficiently the neurologists in Unna were able to assess the extracranial and intracranial arteries independently highlighted the practical advantages of this approach. This experience strongly motivated me to learn neurosonology myself. Although two weeks were clearly insufficient to gain full proficiency, I have since continued practicing and developing this skill in my home institution.</p>
<p>Another particularly striking aspect was the promptness with which patients sought medical attention at the onset of stroke symptoms. In contrast, delayed hospital presentation remains a significant challenge in my country, and late arrival frequently precludes patients from receiving thrombolytic therapy. This experience reinforced my determination to contribute, to the best of my ability, to improving thrombolysis rates and overall stroke care statistics in Georgia through education, system optimization, and interdisciplinary collaboration. Equally noteworthy was the door-to-needle time of 18 minutes for intravenous thrombolysis, which has become an important personal benchmark for my future clinical practice.</p>
<p>I also observed meaningful cultural differences influencing medical decision-making, particularly regarding end-of-life care and patient autonomy. The German approach places strong emphasis on patient self-determination and advance planning, which contrasts in several ways with practices commonly encountered in my home healthcare system. These observations provided valuable perspective and prompted reflection on how cultural context shapes ethical and clinical decisions.</p>

<p>I would like to conclude this report by expressing my appreciation for the town of Unna itself. I was fortunate to visit shortly before Christmas, when the town was beautifully decorated and filled with a warm, festive atmosphere. The welcoming environment of this small town contributed greatly to the overall positive experience of my stay.</p>
<p>Finally, I would like to express my sincere gratitude to the entire neurology team at Christliches Klinikum Unna West for their openness, professionalism, and willingness to share their expertise and tolerate my poor knowledge of German language. I am especially thankful to the Head of the Department, Professor Z. Katsarava, for the leadership, support, and commitment to excellence in stroke care and medical education.</p>
<p>Overall, my participation in the ESO D2D programme at Christliches Klinikum Unna West was both professionally enriching and personally inspiring, and it has had a lasting impact on my approach to stroke care and neurological practice.</p>

<p>First and foremost, I would like to express my heartfelt gratitude to Professor Mazya and his team for the warm welcome, hospitality, patience, enthusiasm, humour, and genuine human attitude — as well as for answering all my questions and for generously sharing their time.</p>
<p>I was deeply impressed by the work of the department — above all, by its logistics, clear division of responsibilities, and system-based organisation. The level of practicality stood out: clear algorithms, tables, and reminders — everything readily available and thoughtfully designed. Workspaces are organised in a way that allows clinicians to be both comfortable and maximally efficient. The technical and material resources are equally impressive — state-of-the-art CT scanners and a cath lab that truly feels like a spacecraft.</p>
<p>Continuous education, meetings, case discussions, team-based decision-making, and multidisciplinary consultations are an integral part of daily routine. The professionalism of the nursing staff is exceptional. However, the most important aspect is how people work. Almost silently. Quickly. Without unnecessary words. Everyone knows their role, performs it precisely, and is able to “read the room”. This approach results in remarkably short door-to-needle and door-to-groin times and, ultimately, better patient outcomes.</p>
<p>I also observed a higher average age of treated patients, milder or well-compensated comorbidities, and a seemingly lower incidence of severe strokes — a reflection of long-terminational investment in primary prevention and the active, healthy lifestyle of the population.</p>
<p>Yes, this is about funding and national health policy. But even more, it is about systems — systems that are built over years, often from within, slowly and sometimes painfully, through difficult and unpopular decisions, persistence, and the courage to defend what is right. And, above all, it is about people — the people who carry these systems every single day. I am deeply grateful to everyone involved in my internship experience for a renewed sense of inspiration to grow, improve, and implement what I have learned in my own clinical practice.</p>
<p>(And to Stockholm as well — a city I truly fell in love with.)</p>
