Your personal information is being collected by Doctors of BC on behalf of the local Divisions of Family Practice for the purpose of working collaboratively with Divisions to enhance local patient care and improve professional satisfaction for physicians. The Divisions will provide Doctors of BC your personal and business contact information, as well as sessional payment information for any subsequent programs you participate in annually or upon request, for ongoing learning and capacity building; for new programs, services and resources development, including evaluation and reporting under the respective funding agreements.
By completing and submitting the above information, you consent to the collection, use, and disclosure of your information for these purposes in accordance with the Personal Information Protection Act.
