Traditionally, medical discharge is very long and complicated, filled with too many details, can easily take up 30 pages

We created a platform for young adults to take control of their medical wellbeing after being discharged from the hospital.

Specifically, our app aims to benefit the users in 3 main ways:

1. Simplify A complicated process into a visually enhanced experience, that is easy and fun to use, entertaining while at the same time educational to 13-18 year olds.

2. Connect Doctor to the patient through FHIR.

3. Build history Traditionally, patients would have to contact the old doctors - a long process. We aim to provide a seamless transition into adult healthcare.
