I used NJII API to access the health care data.
Plan was clear get all relevant data , which included Patient name, address, gender, dob etc Encounters which are patient's visits to hospitals or care provider and All Conditions for which patient was diagnosed for..
This is just enough to suggest probable diet plan for patient under consideration.

I worked on my own, I used Flask Service or website setup and python , html, css and js for rendering data on screen and little designing.
