NYConnects

It’s been a busy and impactful week here at New York Connects as we continue supporting individuals and families in navigating long-term services and supports:


🏠 Housing Help – We helped clients fill out housing applications for affordable and senior housing across New York state, ensuring they had the documents and guidance they needed.

💳 Medicaid Enrollment – We supported older adults and individuals with disabilities in applying for Medicaid, giving them access to vital health coverage and home care options.

💼 Disability Benefits Support – Our team assisted callers with applications for Supplemental Security Income (SSI) and Social Security Disability Insurance (SSDI), walking them through medical documentation and eligibility requirements.

⚖️ Fair Hearing Request – When CDPAP services were unexpectedly canceled, we helped a client file fair hearing requests to challenge the decision and request aid continuing, so services could remain in place while appeals were pending.
