Shinya Tsukomoto might genuinely be struggling mentally, and because of that he absolutely nails all the self destructive tendencies you go through when you're not mentally well. It's especially stark with how Kotoko's character is presented like she doesn't comprehend the realities of the world like others and needs to remind herself about the reality either through the pain of self harm or calmness of singing. It's harrowing and it's terribly bleak but it's one of the most empathetic portrayals of mental illness put on screen.
