International High School Document Submission

This form may only be used by official verifiable school administrators. This form is intended for submission of international high school documents/transcripts. University transcripts and other credit-bearing coursework must arrive in an official manner directly from that institution. For all other materials please follow these guidelines for submitting documents to UC Berkeley:If you are a non-US citizen/permanent resident attending a US high school, do not use this form. Please refer to this page for how to submit your documents.
For security purposes, do not include any private information in this form such as credit card, social security numbers, or private health information.
