Angel Flight Central wants to help you by providing charitable air transportation!
Please read through this very thoroughly as the success of your flight depends on it.
AFC serves people with a financial need who reside in the Midwest by arranging charitable flights of 100-900 nautical miles for patients accessing medical care. AFC also coordinates compassion, humanitarian, and disaster relief and response flights on a case by case basis. Our pilot volunteers fly small, general aviation aircraft with typically 1-3 available seats. There is no medical personnel or equipment on board the aircraft.
AFC is a recipient of grant funded commercial airline passes from Southwest Airlines. Commercial travel is reserved for medical travelers going more than 900 nautical miles from their home for treatment and where Southwest terminals are available. This resource is specifically for medical travelers and cannot be used for relocation purposes, Compassion or Humanitarian flights, or international travel.
Advance Notice - You must complete a flight request form and submit all of the requested information at least 7-10 business days prior to your flight date. A Flight Coordinator will contact you within 1-2 business days to discuss your flight needs and determine if you qualify. Be sure to leave a phone number that we can easily reach you at and check to see if your voicemail box is full. We’ll likely be calling from a number you don’t recognize.
Confirmable Appointment Date – Whether you are going to a one-time appointment or have a series of appointments or procedures, you must provide confirmation of the appointment date for the flight you are requesting. A screenshot, forwarded email confirmation from your doctor’s office, or the name and phone number of someone who can confirm your appointment is required. If you aren’t sure of your appointment date yet, that’s ok! Simply let us know where you are in the process when we contact you.
Medical Consent – Include your primary physician’s name, phone and fax numbers, and an email address or the name of a care coordinator or assistant that can help get our Medical Release form to your doctor. If your flight companion or caregiver has medical complications, we are required to collect a Medical Release for them and will need you to provide the appropriate information. Flights will be cancelled if we are unable to obtain the medical release prior to the flight date.
Back-Up Plan – AFC is unable to guarantee flights due to factors outside of our control. Weather can cancel flights; an unexpected plane maintenance issue could be the cause; any situation that would make the flight unsafe will result in a cancellation. If we must cancel, you are responsible for your own back-up plan.
Physical Capabilities – General Aviation passengers must be able to walk, climb stairs (lifting the leg up to 18-20 inches) and pull yourself into the seat of the aircraft unassisted. Children under 40lbs must be in an approved car seat. Please watch the video in the Passengers section of our website.
Baggage – General Aviation passengers are limited to 50lbs total baggage combined. If you have medical equipment or fluids that you need to carry with you that would quickly exceed the baggage limit, call the office to discuss. We must have the dimensions, weight, and other specifics of all walkers, wheelchairs, or other equipment.
Identification – Current legal government issued I.D. is required to board any flight arranged by AFC. AFC cannot fly undocumented residents.
