At Access Care Partners, our mission is to preserve the dignity, independence, and quality of life of older adults and persons with disabilities and chronic conditions OF ALL AGES. We offer a wide range of services to support both caregivers and people receiving care, including:
We assist individuals with chronic conditions or disabilities by helping them hire and supervise Personal Care Attendants.
We offer healthy, well-balanced meals prepared locally and delivered to your home or served in community settings.
Our Home Care Services support independence at home, offering personal care, housekeeping, and much more.
We support caregivers and grandparents 55+ raising grandchildren by providing information on resources, support, training and respite.
Our Program provides a monthly stipend to caregivers caring for older adults or individuals with disabilities at home.
Our trained volunteers assist older adults with managing finances, including writing checks and balancing checkbooks.
If you are an older adult, an individual with disabilities in need of assistance with household tasks or your daily activities, or a caregiver in need of support, contact us to learn about your options regardless of your insurance or income. We offer many programs like Home Care, home delivered meals, and other services which are not limited to MassHealth members.
Access Care Partners’ main service areas include Holyoke, Chicopee, Granby, South Hadley, Ludlow, Belchertown, Ware, and many other surrounding communities throughout Western Massachusetts.
