Information and Referrals
Get connected to the resources you need. Our Navigators provide personalized information and referrals for services, programs, and supports in the community.
Assistive Tech / Medical Equipment
Our program provides individuals with disability-related, donated equipment at no cost. Please note that equipment availability, colors, brands, styles may vary, and are donated items. We also accept donations of durable medical equipment. All items are cleaned and sanitized by our staff before lending.
Home Accessibility
Our Home Accessibility Program completes home modifications projects through grant funding to make homes for people with disabilities more accessible.
Projects include:
- Ramps (aluminum or wooden)
- Low-rise steps
- Stairlifts
- Grab-bars
Northwest Ohio Medical Supply Closet
Through our partnerships with the Area Office on Aging and Principle Business Enterprises, Inc., we provide free adult incontinence supplies each month.
Pick up supplies on the first Wednesday of every month from 11 AM to 3 PM at the Sylvania Senior Center, located at 7140 W Sylvania Ave, Sylvania, OH 43560.
