This program is intended for homeless and low-income families and individuals in the Five Cities area who are in need of support to avoid loss of housing or experiencing unsheltered homelessness. 5CHC has limited funds to help people overcome barriers that enable them to move toward self-sufficiency and increased stability. These funds are prioritized for clients working in direct case management with 5CHC to gain or maintain permanent, stable housing. Examples include assistance with utility bills, auto repairs/registration (when cars are used for work or to live in), clothing, employment readiness, and transportation resources. 5CHC maintains a list of local partners that also help with these costs and may refer to these programs first before issuing funds directly to a service. Funds are never given directly to individuals.

Please note that clients need to complete a Coordinated Entry to be eligible for these funds.
